Rules & Regulations

NO PURCHASE NECESSARY TO ENTER OR WIN GRANT.
ELIGIBILITY: The drawing is open to legal residents of the U.S., who are 25 years of age and older. Employees of the Reproductive Wellness Clinic, its affiliates, subsidiaries, members, promotion and advertising agencies, subsidiaries and related companies and their immediate families (spouse, and parents, children and siblings, and each of their respective spouses regardless of residence) and household members of each, whether related or not, are ineligible.

1. HOW TO ENTER: To enter without purchase or obligation, sign up online or hand print your name, address, city, state, zip, day and evening phone numbers, email address and the words Reproductive Wellness Fertility Treatment Grant along with a essay on why your entry should be chosen, and mail in a stamped envelope to: Reproductive Wellness Clinic, 2425 Camino Del Rio South, Suite 150 San Diego, CA 92108. Mail-in entries must be received by October 16th, 2010. Limit one entry per person or e-mail address throughout the duration of the drawing. Entries may be subject to verification prior to drawing. All entries become the property of Reproductive Wellness Clinic and will not be returned or acknowledged.

2. PRIZE: The Reproductive Wellness Clinic will pay for a complete (up to 9 months) Oriental Medical Natural treatment program (Acupuncture & Botanical Medicines) FREE of CHARGE a value of over $9000.

3. ODDS: The actual odds of winning or being eligible either for the grand prize or secondary prizes will depend upon the total number of eligible entries received and the merit of each individual application as assessed by the judging panel.

4.RESTRICTIONS/CONDITIONS: The judging panel decision will be final, no substitutions or cash equivalents will be provided. Patient must live within the greater San Diego CA (with in approximately 25 miles to either clinic) area and be able to travel independently to one of the Reproductive Wellness clinic locations. In the event of a Grand Prize winner the duration of “free” treatment shall not exceed 9 months or $9,000.Grant is non-transferable and non-refundable. No substitution or cash equivalent of grant is permitted. Federal, state and local taxes, if any, and any other costs or expenses related to acceptance and use of the prize not specified including gratuities, are the sole responsibility of the winner. The Reproductive Wellness Clinic will notify winner after receipt and verification of Eligibility. Grant will be awarded, approximately 6-8 weeks after notification by the Reproductive Wellness Clinic. Winner must take delivery within 30 days of notification of availability or grant will be forfeited and may be awarded to an alternate winner.

5. DRAWING: Grant will be awarded on or about November 1, 2010 by Reproductive Wellness Clinic, 2425 Camino Del Rio South, Suite 150 San Diego, CA 92108 from among all eligible entries received. The selection committee is comprised of the entire Reproductive Wellness team of practitioners and staff who will collectively select the winning applicant.

6. NOTIFICATION OF WINNER/AWARDING OF GRANT: The winner will be notified by phone and/or email within 21 days of notification.

7. LIMITATIONS OF LIABILITY: The Reproductive Wellness Clinic is not responsible for any late, lost, incomplete or misdirected entries, or responsible for incorrect or inaccurate information, whether caused by Web site users or by any of the equipment or programming associated with or utilized in the grant drawing; any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access of the Web site; any injury or damage to participants or to any other person's computer(s) related to or resulting from participating in this prize drawing, or downloading materials from or use of the Web site. If, for any reason the prize drawing cannot be conducted as planned by reason of infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Reproductive Wellness Clinic which, in the sole opinion of Reproductive Wellness Clinic, corrupt or affect the administration, security, fairness, integrity or proper conduct of this prize drawing, Reproductive Wellness Clinic reserves the right, in their sole discretion, to cancel, terminate, modify or suspend the drawing and may conduct drawing and select a winner from among all eligible entries received for the drawing prior to the action taken. In no event will Reproductive Wellness Clinic be responsible or liable for any damages or losses of any kind, including direct, indirect, incidental, consequential or punitive damages arising out of access to and use of the Web site reproductivewellness.com, or the downloading from and/or printing material downloaded from said site. Without limiting the foregoing, everything on the site is provided "as is" without warranty of any kind, either expressed or implied, including but not limited to the implied warranties of merchantability, fitness for a particular purpose or non-infringement. Some jurisdictions may not allow limitations or the exclusions of liability for incidental or consequential damages, or exclusion of implied warranties, so some of the aforementioned limitations or exclusions may not apply. 8.
